Vaccine & Testing Requirements

Vaccination requirements and laboratory testing for citizenship and completion of USCIS applications are based on CDC guidelines and recommendations for Civil Surgeons.

Mandatory Requirements

Laboratory Testing

Gonorrhea

Urine test required for age-appropriate applicants.

Syphilis

Blood test equired for age-appropriate applicants.

Tuberculosis [Quantiferon or T-spot]

Blood testing is required for all applicants age 2 and older.


Covid & Flu Vaccines

Covid-19

** The Covid-19 vaccine is no longer required as of January 22, 2025 **

Influenza [Flu]

Required between October 1st to March 31st.


Mandatory Adult Vaccines (Ages 18-64)

Measles | Mumps | Rubella [MMR]

Two doses are required for completed series.

Varicella [Chickenpox]

Two doses are required for completed series or varicella history.

Tetanus/Diphtheria [Td or Tdap]

One dose is required every 10 years at minimum.

Hepatitis B (required until age 59)

Three doses are required unless Heplisav-B two dose series.

Polio Vaccine [**New 2024 Requirement**]

Three doses are required for completed series.


Mandatory Adult Vaccines (Age 65 and older)

Pneumococcal [Pneumonia]

One dose required if PCV20. See CDC guidelines if other versions used.

Varicella [Chickenpox]

Two doses are required for completed series or varicella history.

Tetanus/Diphtheria [Td or Tdap]

One dose is required every 10 years at minimum.

Polio Vaccine [**New 2024 Requirement**]

 Three doses are required for completed series. 

**A completed series is not required if the first dose of the vaccination series is obtained within 1 month of the medical exam.**

Waivers are requested through USCIS and are not given by the Civil Surgeon.
